Application Forms

You can access the online application by clicking the "Apply Now" button below. Please make sure to submit your application and the $75 fee by December 16, 2024. Applications will be available starting September 1, 2024.

To receive a decision by Friday, March 7, 2025, complete your initial application and submit all required documents listed below by January 31st, 2025:

Teacher Recommendations
The HAIS Math and English Recommendation forms can be requested through the application portal. Your student’s teachers will receive an email with a link to the form once the request is submitted, which they can complete the recommendation and upload directly to the student’s application. Teachers are also welcome to email their recommendation to admissions@incarnateword.org. Download the forms below if you would like to request your recommendations in-person or email.

Middle School Transcripts
Submit transcripts for the previous two grade levels and the first semester of the current grade level. Upload a PDF of your student’s 6th, 7th, and first semester of 8th grade school reports directly on the application portal. Request that your student's first semester grades be sent by their current school to admissions@incarnateword.org in January. All transcripts are due by January 31, 2025. Please utilize the transcript release form below if needed.

Independent School Entrance Exam (ISEE) Required
All freshman applicants are required to submit scores from the Independent School Entrance Exam with their application. Register to take the ISEE at IWA on Sat. January 11th at 8:00 am at iseeonline.erblearn.org using code: KNBAXVYC.

This exam can be taken online or in person at various locations.

To report your ISEE score to Incarnate Word Academy, use school code 570451. This will ensure your score will be sent to IWA. Transfer applicants can submit PSAT, SAT, ACT, and other standardized test scores they have available. ISEE scores can only be taken once per season. Students can take it once in the Fall ( August - November) and Winter (December-March). Students can submit two ISEE scores. Scores can also be sent directly to admissions@incarnateword.org. Register at iseeonline.erblearn.org.

Birth Certificate
All applicants have their age verified by submitting a birth certificate on the application portal.

Application Forms
Students and families can access the Transfer application below. We encourage students and parents to submit their application as soon as they are interested to provide our committee adequate time to review your daughter's application. After completing your daughter's application, it can take up to 1-2 weeks to deliver an admissions decision. Transfer students in grades 9th - 10th will be considered with the exception of students grades 9th -11th who are relocating to Houston, Texas.

Teacher Recommendations
Two teacher recommendations (Math and English HAIS forms) are required for admission consideration. Print the forms below and give them to your daughter’s current Math and English teachers for completion.

Transcripts
Transcripts from the previous two grades and first semester of the current grade are required. Please print the records release form and submit to any relevant schools for completion. E-mail transcripts to admissions@incarnateword.org.

Entrance Exam
All applicants must submit standardized test scores such as the PSAT 8/9, PSAT 10, ACT and SAT scores are accepted at IWA. Transfer applicants who have completed the ISEE in 8th grade may submit their test scores for consideration.

*International Transfer Applicants are welcome to submit TOEFL scores*

Interview
All transfer students are required to interview. Once the application and all supporting documents are received by the Admissions Office you will be contacted to set up an interview time.
